Information, Referral & Assistance: certified specialists listen carefully to your needs and provide you with customized, unbiased referral options to help you make informed decisions.

Options Counseling: helps you understand services available to address your needs and assess eligibility for programs. Options Counselors help you develop an action plan to reach your goals and follow up to ensure you receive the right level of support.

Heritage also funds community resources

Assisted Transportation: offers door-to-door assistance if you have physical or cognitive difficulties using regular vehicular transportation like cars, vans or buses.

Transportation: transports you when requested from one location to another via vehicle, but does not include any other activity. Sometimes called “curb-to-curb” service, it differs from Assisted Transportation in that providers may not be able to help passengers get from their door into the vehicle and vice versa.
