Options Counseling

Options Counselors are qualified and knowledgeable professionals that will visit with you via phone, or in person as often as you need, to help connect you to services and plan for potential future needs.

Options Counselors help empower individuals to make informed choices about care services by providing personalized assistance with planning and accessing services. Options Counselors through the LifeLong Links system are able to address many of the frustrations that consumers and their families face when trying to link information such as resources for planning future long-term care needs and coordinating with existing aging and disability service systems. Options Counseling is a service to help create a person-centered, community-based environment that promotes independence and dignity for individuals.

Options Counseling is a service that will:

  • Provides an interactive decision-making support process

  • Provides unbiased information

  • Ensures all possibilities are considered

  • Provides for individual planning for services and supports

  • Assists in navigating through the array of services

  • Provides Application Assistance

  • Create a plan for future needs

Any older adult (60+) or a person with a disability (18+) who is a resident of Benton, Cedar, Iowa, Jones, Johnson, Linn or Washington Counties is eligible for this program.
