The Case Management Program includes the assessment and coordination of home and community based services. This service is also called the Case Management Program for the Frail Elderly (CMPFE).
The Case Management Program for the Frail Elderly (CMPFE) is "… a comprehensive system that strives to provide Iowa’s frail elderly with the opportunity to make their own choices regarding long-term care and to receive home and community based service in the home and community setting." Who is eligible?
A consumer must: 1) be a resident of Iowa, 2) 60+, 3) have a need of two or more long-term care services, and 4) have a need for coordination of those services.
In most cases, the service of case management is provided at no charge to the client but the service of case management and the services the case manager sets up will have a fee. Payment for services is often based on individual need and eligibility criteria of the funding source. Funding can come from multiple sources including; Private Pay, Sliding Fee Schedules, Medicare, and/or Title XIX Medicaid - Elderly Waiver.
Approximately 700 people in our seven county region are served by the Case Management program through Heritage Area Agency on Aging.
Many people simply could not access the services they need or identify funding for those services without the help of a skilled case manager. Without Case Management it would be difficult for many frail seniors to even consider staying in their own homes.
